DSWD Financial Assistance Guide and Tips

Having a family member admitted in hospital can be considered as a crisis. A lot money is expected to be used especially if you are in a private hospital like Makati Medical Center. Even if you are in a private hospital, you can still get some financial assistance from government institutions like the DSWD to help offset your hospital bills.  I have written a step-by-step order to help save you some time and effort. Kindly make sure the following requirements are complete to avoid any issues.

Here is a guide to getting some financial assistance from DSWD:

1. Latest Clinical/Medical Abstract (original or certified true copy) – this usually takes 3 working days to produce that is why it is important to ask your doctor for it, once your relative or loved one is admitted in the hospital.

2. Referral/Endorsement Letter from the Hospital’s Social Service Unit (electronically generated to be given by staff)

3. Latest Statement of Account/Hospital Bill (original and validated by Credit/Billing officer)

4. Barangay Certificate of Indigency of the Patient (original)

5. Barangay Certificate of Indigency of the Relative who will process the application (original)

6. Photocopy of any Government Issued ID of Patient (front and back)

7. Photocopy of any Government Issued ID of Relative who will process the application (front and back)

8. Prescription (for medicine)

9. Laboratory Request/Medical Procedures (recommended by the attending Physician)

10. Social Case Study Report from the Local Social Welfare Office in the city or municipality hall where the patient lives –  for the Social Case Study Report you will also need some other requirements, see picture below for more information. (optional only, needed only for amounts greater than P5,000).

Once you have received the Guarantee Letter, you must go to the Hospital that the patient is admitted and have them receive it. Make sure you have a receiving copy. After that you must go back to DSWD CIS department and have them received it also. Just make sure you have a receiving copy for your safety and reference.

If you are in a Private Hospital, the money from DSWD is for reimbursement, the processing time is 3 – 6 months. You can follow it up after a month, just in case the check will be processed sooner. You can issue a Promissory note up to 100 thousand to cover the hospital bills depending on your agreement with billing department since you already have a Guarantee Letter from the DSWD.
